These are typically children that have additional support needs and a significant delay in two or more areas of development impacting their daily function. The service provides specialist coordinated support to enable children to meet their optimum outcomes and supports effective transition into early education.
The CDC service provides targeted multi-agency support to children and their families through attendance at sessions in the centres, and by providing individual work within the child’s home or local community through the community caseload and outreach. When referrals are accepted a robust assessment is carried out and an individual plan of support is developed. The CDC service works closely with Education as part of the Pre-School Community Team (PSCT) and works closely with the multi-agency team around the child particularly Health Visitors, Speech and Language Therapy and Psychology.
