Referral routes
Referrals accepted via online referral hub from Fife Health and Social Care and from Care Home health care professionals. There is no self referral for this service. Please ask the health care professional looking after you to make the referral as this is current referral process.

Community tissue viability service
Our service supports community hospitals, hospices, Fife’s care homes, district nurses, treatment rooms and GPs by providing education and local improvement initiatives by using evidence based practices and pathways of care. We also co-ordinate the provision of specialist pressure relieving equipment from Fife’s Community Loan Store alongside Fife Council.
What we do
The team provides pressure ulcer and complex wound assessment, advice, prevention and management. We are responsible for ensuring the quality of patient care is reviewed, assessed, implemented and monitored to maintain standards of care for patients and their families.
In addition, we are responsible for the clinical audit of specialist nursing services for Community Tissue Viability to ensure evidence based practice monitoring of standards of care within the defined policies.
