The MND Clinical Nurse Specialist is ideally placed to ensure a proactive approach in the delivery of timely advice and support for evidence-based patient-centred care. They act as the patient advocate, are a point of reference, support, education and guidance.
Palliative care for people living with MND starts at the time of diagnosis. This care continues throughout the disease trajectory and is crucial to care management at all stages of the disease process offering symptom control and best supportive care.
The specialist approach to care for people living with MND enables people living with the disease, together with their families, to retain as good a quality of life as possible, promoting abilities and maintaining independence.
Referral routes
All patients in Fife living with a diagnosis of MND have a named Clinical Nurse Specialist at diagnosis and will be reviewed on a regular basis.
Patients living in North East Fife are covered by NHS Tayside and can contact their Clinical Nurse Specialist on 01382 425726.
Patients living in Kincardine, West of Fife, Dunfermline, central Fife, Kirkcaldy, Glenrothes and Levenmouth are covered by NHS Fife and can contact their Clinical Nurse Specialist on 01592 643355 Ext 20621.
The service in Fife is provided by a part-time CNS. If you are unable to speak directly with the CNS when you phone, please follow the directions contained within their telephone answer message.Â
Motor Neuron Disease (MND) Specialist Nurse-led Service
The MND Specialist Nurse-led Service is a small team.
What we do
The CNSs are able to support patients with:-
- Co-coordinating patient care by liaising with members of the Multi-Disciplinary Team including Neurology Consultants, GPs, District Nurses, all Allied Healthcare Professionals, Social Work, community Occupational Therapy, Specialist Palliative Care and supporting charities
- Onward referrals to appropriate services
- Onward referrals for aids and adaptations and support for re-housing
- Discussions around timely Advance Care Planning, including guidance on interventions which can be offered and documenting and sharing patients’ wishes
- Joint home visits with appropriate members of the wider MDT
- Prescribing Blue Badges and assistance with completion of paperwork for parking permit spaces
- Symptom management advice and support
- Assistance with accessing grant opportunities from supporting charities for various support such as well-being, equipment including riser-recliner chairs, adaptations and time-out opportunities
- Onward referral for benefits and welfare, as well as Advocacy support
- Recruitment to Clinical Trials and research opportunities for people living with MND, including taking research consent
- To provide support and education to patients, families, carers and members of the wider MDT
